What is endometriosis?

Endometriosis is when some of the cells similar to those that line the uterus (womb), called the endometrium, grow in other parts of the body. They usually grow in the pelvis, but can move to almost any part of the body.

Even though they are not in the uterus, the cells still respond to messages from the ovaries. The endometriosis tissue gets full every month and then bleeds when the woman has a period. Over time, this process can lead to inflammation and scarring, and can cause organs to stick together in places (known as adhesions).

Endometriosis is a progressive, chronic condition. It can be very painful, it can affect fertility and it can prevent women from participating fully in education, work or sporting activities. About 1 in 9 women in Australia develop endometriosis by their 40s, and the condition causes tens of thousands of hospitalisations every year.

Acupuncture for Endometriosis Research

A systematic review and meta-analysis of complementary treatments for women with symptomatic endometriosis was published in 2018. It concluded that numerous complementary treatments have been used to alleviate the symptoms of endometriosis, but only Acupuncture has demonstrated a significant improvement in outcomes. [1]

Acupuncture is recommended in the Endometriosis Australian Clinical Practice Guidelines for the diagnosis and management of Endometriosis. (low to moderate quality evidence).

A recent study from 2017 titled the “Effects of acupuncture for the treatment of endometriosis-related pain: A systematic review and meta-analysis” was undertaken to determine the effectiveness of acupuncture. It concluded that Acupuncture reduces Endometriosis related pain and serum CA-125 levels, regardless of the control intervention used. [2]

The above research suggests that Acupuncture may provide beneficial outcomes however other papers did not have the same findings so the evidence is regarded as mixed.

So what exactly is CA-125 and how does it relate to Endometriosis?

In normal women, plasma concentrations of CA125 are increased slightly at ovulation and significantly during menstruation. Marked increases are observed during pregnancy and following peritoneal irritation by infection or surgery. These data are consistent with the concept that CA125 in normal women is mainly derived from the endometrium and the irritated peritoneum. Plasma concentrations of CA125 are markedly elevated in women with cystic ovarian endometriosis and/or deeply infiltrating endometriosis, but not, or only slightly, in the luteal phase of women with minimal or mild endometriosis.

This is consistent with the recent concept which considers minimal endometriosis as a normal condition occurring intermittently in many women, in contrast with deep endometriosis and cystic ovarian endometriosis which are called ‘endometriotic disease’.

Serum CA125 is not a good marker for endometriosis but it is a helpful additional parameter to diagnose endometriotic disease in patients with chronic pelvic pain. Following treatment of endometriosis, elevated plasma concentrations of CA125 could be used as an argument that treatment has been incomplete, or that the condition has recurred. Assaying CA125 in peritoneal fluid requires high sample dilutions or a modified immunoradiometric assay, and until now, its clinical value has been questionable. [3]
